Only what the form asks for.
When you submit the contact form we receive: your name, your relationship to the child, your phone number, your email address, your city, how you prefer to be contacted, how you heard about us, the child's age, which services you ticked, the schedule and start time you are after, your insurance carrier, plan name and plan type, and anything you type into the questions box.
We deliberately do not ask for a member ID, a policy or group number, a policyholder's details, a diagnosis, a diagnosing provider, medical history, or your child's name. Those are collected later, by phone or through CentralReach, our secure records system, once you have decided to work with us.
Our web host keeps standard server logs, which include the IP address a request came from. We use those only to limit spam submissions and to keep the site running.
Answer you, and check your benefits.
Form submissions are emailed to our office inbox. We use what you send to reply to your enquiry, to verify your insurance benefits if you asked us to, and to keep a record of the conversation. If you gave us an email address, we send you a short confirmation that we received your message.
We do not sell or rent your information. We do not share it with advertisers or data brokers. We share it outside our team only where we need to in order to do what you asked (for example, contacting your insurer to verify benefits) or where the law requires it.
Into a mailbox covered by a HIPAA agreement.
Our email runs on Google Workspace, and we hold a Business Associate Agreement with Google. That is the contract HIPAA requires before a provider can use an outside service for protected health information. It means that once your message reaches our inbox it is stored under those terms: encrypted at rest on Google's systems, subject to the same safeguards as the rest of our records, and not used by Google for advertising.
The journey to that inbox is a different matter, and worth being straight about. Mail between providers is normally encrypted in transit, and where both ends support it that connection is strong. It is not guaranteed end to end the way a portal login is, because your own email provider, and any service you send from, sits in the middle. Nobody can promise you otherwise about email in general, and you should be wary of anyone who does.
That is why this form asks for so little. Please do not send medical records, diagnostic reports, photos of insurance cards or anything similar through the form itself. Once we are talking we will give you a secure way to share them, and the paperwork gets handled properly from there.
Once you become a client, everything we hold about your child is protected health information, handled under our HIPAA notice of privacy practices, which we provide during intake. That is a stricter set of rules than this website policy, and it is the one that governs your child's record.
What runs on these pages.
This site does not use advertising pixels or cross-site trackers. There is no analytics profile built on you here, and nothing on these pages targets you with ads based on what you read.
If we add analytics later, we will say so on this page and choose a tool that does not build advertising profiles from visits to a healthcare site.
This site also loads its typefaces from Google Fonts, which means Google's font servers receive the request. That is how the fonts arrive on your screen.
Changing your mind, and getting your information back.
You can ask us at any time to tell you what we hold from your enquiry, to correct it, or to delete it. Email mckinney@successonthespectrum.com or call (972) 442-9971 and we will take care of it.
If you asked us to contact you and change your mind, say so and we will stop. Texas residents have rights under the Texas Data Privacy and Security Act, including the right to know what personal data a business holds about them, to correct it, to delete it, and to opt out of its sale. We do not sell personal data at all.
This site is for parents and caregivers.
These pages are written for adults. We do not knowingly collect information directly from children under 13. Everything on this form is intended to be completed by a parent, guardian or other adult acting for a child.

We link to successonthespectrum.com, to state and federal resources, and to a small number of other organisations we think are useful. Once you follow a link you are on their site and their privacy policy applies, not ours.
If this policy changes.
We will update this page and change the date below. Material changes to how we use enquiry information will be described plainly rather than buried.
